Client Contractual Agreement
I ______________________________________, certify that I am the owner of this/these pet(s), and grant permission to Blythe or Robert Groff to obtain on my behalf and in my pet’s best interest the veterinary care necessary to treat any illness or injury. I agree to pay all veterinary and other necessary services incurred by my pet(s) during its stay.
Vaccinations: We require vaccinations for all dogs. These vaccinations include Parvo, Distemper, Rabies, and Bordetella. It is the owner’s responsibility to maintain the currency of these vaccinations.
Flea Control: All dogs must be on flea prevention from April 1st thru November 30th of each year. The type of antiparasitic is determined by the owner, but may NOT include a flea/tick collar. If a dog is found to have fleas at anytime he/she will be given a bath and flea treatment at the owners expense (see prices below).
Health: All dogs must be in good health. Owners certify that their dog(s) is/are in good health and has not been ill in the last 21 days. On admission all dogs must be free from any condition which could potentially jeopardize other dogs. Dogs who have been ill with a communicable condition in the last 21 days will require a veterinarian certification of health to be admitted or re admitted.
Scratch and Dent: I realize that dogs will occasionally get nicks, scratches, bruises and other "dings," and I agree that Blythe and Robert Groff are not responsible for these injuries.
Overnight Care Fees: Payment for overnight care is due at the time of pick-up.
Behavior Issues: I agree that my dog(s) have not harmed nor shown aggression or threatening behavior towards a dog or person.
I understand that I am solely responsible for any harm caused by my dog(s) while my dog(s) are under Blythe and Robert Groff’s care. I also assume full financial responsibility for any and all expenses involved with damages caused by my dog(s) to kennels, property, or persons’ associated with Blythe and Robert Groff, and their facility.
I further understand and agree that Blythe and Robert Groff and any staff and volunteers, will not be liable for any problems that develop, and I hereby release them of any liability of any kind arising from my dogs(s) attendance with Blythe and Robert Groff. Blythe and Robert Groff assumes no liability for loss or damage from medicinal applications, disease, sickness, death, running away, theft, fire, tornado, injury to persons, other pets or property by my dog(s) or other unavoidable causes, due diligence and care having been exercised.
All dogs are confined at night for their own safety.
This is not a licenced and insured boarding facility
Charges and Terms:
$20.00 a day, a dog.
$10.00 flea treatment
Bath: $10.00 for dogs under 15lbs.
$15.00 for dogs 15- 30lbs. $20.00 for dogs over 30lbs
Toenails: $7.00 Nails Clipped and Filed: $10.00
Ear or Eye Cleaning: $3.00 Teeth Brushing: $3.00
(Blythe is not a certified groomer, but can provide basic hygienic care).
Please call before picking up your dog, or set a pick up time before departure, to ensure we are home.
Dogs being dropped off before 5:00p.m. will be charged for the entire day.
Dogs need to be picked up before noon, or the owner will be charged for the entire day.
A cancellation of at least 2 weeks is required over holidays ( Christmas, Thanksgiving, Easter, Independence Day, and Memorial Day), as well as Spring Break, and during Graduation Season. If advanced notice is not given, you will be charged half the cost of what your dogs(s) stay would have been.
Your signature indicates you have read and understand the payment terms, rules, and agreements. I agree to abide by the rules, regulations, and accept all the terms, conditions, and statements of this legally binding agreement.
This legal binding contractual agreement applies to any and all stays by your dog(s) and doesn’t expire.
